Common mistakes

Filling out the NYC Buildings OP128 form, which is an Ordinary Plumbing Work Report, is crucial for compliance and accuracy in documenting plumbing work. However, several mistakes often occur during this process:

  1. Not typing the form: Although it appears simple, the requirement that the application must be typewritten is frequently overlooked. Handwritten forms are not accepted, leading to the delay of your filing process.

  2. Failing to file double-sided: The mandate that applications must be double-sided is another detail that applicants often miss. This mistake can lead to more paperwork and potentially slow down the approval process.

  3. Incorrect filing status: On the form, applicants must indicate whether it is an Initial Filing or a Correction. Mixing these up or leaving the section blank can cause confusion and delays.

  4. Applicant information errors: Applicant Information is Required for all applications. Missing fields or incorrect information in this section, especially incorrect or outdated contact information, can significantly delay the process.

Documents used along the form

When working on plumbing projects in New York City, particularly those that necessitate the submission of the OP128: Ordinary Plumbing Work Report, individuals often find that additional documents and forms are required to complete the process effectively. Understanding each of these documents can help streamline the process and ensure compliance with all relevant regulations and requirements.

  • DOB Permit Application (PW1): This form is used to apply for permits related to construction, plumbing, and other work types. It's essential for projects that go beyond ordinary repairs or replacements that the OP128 form covers.
  • Asbestos Assessment Report: Prior to any plumbing or building work, an asbestos assessment may be required to ensure the safety of the environment and compliance with city health regulations.
  • Limited Alteration Application (LAA1): For certain minor changes that don't affect the use, egress, or occupancy of a space, a Limited Alteration Application may be necessary. This is relevant in cases where backflow prevention devices need to be installed or replaced, as noted in the OP128 instructions.
  • Gas Authorization Form: When a project involves gas plumbing work, including installations, repairs, or replacements, this form is required to confirm authorization from the utility company or appropriate authority to proceed with gas-related work.
  • Annual Boiler Inspection Report: If the project includes the repair, replacement, or installation of boilers, an annual inspection report may be necessary to certify the boiler's compliance with safety standards.
  • Plumbing Inspection Request Form: After completing the plumbing work reported on the OP128 form, a plumbing inspection request form is typically submitted to have the Department of Buildings inspect the completed work to ensure it meets code requirements.
  • Energy Conservation Code Compliance (EN1): For new installations or major updates, compliance with the New York City Energy Conservation Code may be required. This document demonstrates that the work complies with energy efficiency standards.
  • Water Meter Permit Application: If the plumbing work involves the installation or replacement of a water meter, a permit application must be submitted to the Department of Environmental Protection.
  • Work Completion and Sign-off Form: Once all work is completed, this form is used to certify that the project complies with all applicable codes and permits. It's vital for officially closing out the permit application process.

Understanding these documents ensures that plumbing work, particularly that which does not require permits as outlined in the OP128 form, is conducted within the legal frameworks and standards set by the city. Each form plays a critical role in the regulatory landscape of construction and plumbing in New York City, helping to maintain a safe and compliant environment for all residents.

Similar forms

The Permit Application for Plumbing Work shares similarities with the OP128 form, particularly in its requirement for detailed applicant and property information. Both documents necessitate the plumber's contact details, the project location, and specific work descriptions. However, while the OP128 focuses on ordinary plumbing work not requiring permits, the Permit Application for Plumbing Work usually covers a broader scope of projects, including those that need official approval before commencement.

Building Inspection Report forms resonate with OP128 through the detail on work performed and compliance assurance. Both documents require information about the work undertaken, adherence to codes, and the applicant's affirmation of correctness and compliance. The Building Inspection Report, however, extends to a wider range of construction activities beyond plumbing and may involve evaluations by city inspectors.

The Limited Alteration Application (LAA) bears similarities with the OP128 form by offering a streamlined process for minor alterations, including specific plumbing works. Like the OP128, the LAA requires details about the work, applicant, and property, but it is explicitly used for alterations that do not change the use, egress, or occupancy of a building. This contrasts with the OP128's specific focus on ordinary plumbing updates.

The Gas Authorization Application is similar to the specific sections of the OP128 form where gas-related work is detailed, such as the replacement of gas boilers or heaters. Both documents necessitate detailed project descriptions, applicant qualifications, and adherence to safety standards. However, the Gas Authorization Application focuses exclusively on gas work, encompassing a range of projects broader than the OP128's plumbing scope.

The Asbestos Project Notification Form aligns with the OP128 form in its regulatory compliance aspect, especially concerning safety and environmental protection. While the OP128 includes notifications related to plumbing work possibly impacting asbestos, the Asbestos Project Notification form is specifically designed for projects that disturb asbestos-containing materials. It focuses on safety protocols, environmental impact, and adherence to regulations.

The Construction Permit Application shares a broader likeness with the OP128 form, allowing for a wide range of construction activities, including plumbing work. Both forms require detailed project information, applicant credentials, and property specifics. The Construction Permit Application, however, is more encompassing, applicable to a variety of projects beyond the ordinary plumbing work scope of the OP128.

The Backflow Prevention Device Test Report parallels the OP128 form in its focus on specific plumbing components — in this case, backflow preventers. Both documents require detailed descriptions of the plumbing work conducted, certifications by licensed professionals, and the maintenance of standards for public health and safety. The Backflow Prevention Device Test Report specifically addresses the testing and maintenance of backflow preventers to prevent water contamination.

Misconceptions

There are several misconceptions about the NYC Buildings OP128 form, an Ordinary Plumbing Work Report, essential for licensed plumbers in New York City. Understanding these misconceptions is vital for industry professionals to ensure compliance and streamline their filing processes.

  • Misconception 1: The OP128 Form Requires a Permit for Ordinary Plumbing Work

Many believe that filling out the OP128 form means that ordinary plumbing work requires a permit. However, this form specifically caters to plumbing work outlined under section 28-105.4.4 of the Administrative Code, which does not require a permit. The purpose of the OP128 is to report plumbing work deemed ordinary, allowing licensed plumbers to document work without navigating the permit process.

  • Misconception 2: Any Plumbing Work Can Be Reported on the OP128 Form

A common misunderstanding is that the OP128 form covers all types of plumbing work. In reality, this form is designed exclusively for specific types of work, such as the repair or replacement of non-gas piping less than 10 feet long, replacement of minor fixtures, and similar tasks listed under section 28-105.4.4. Work outside these parameters requires different documentation and potentially a permit.

  • Misconception 3: The Form is Complicated and Time-Consuming to File

Some plumbers are under the impression that the OP128 form is burdensome and complex. However, the form is straightforward, requiring basic information about the applicant, work location, and details of the plumbing activities carried out. By providing clear instructions and stipulating the need for typewritten, double-sided applications, the form simplifies the submission process.

  • Misconception 4: The OP128 Form is Only for Residential Properties

There is a misconception that the OP128 form is exclusive to residential properties. While certain parts of the form apply primarily to residential buildings, such as work in buildings occupied by fewer than six families, the form encompasses ordinary plumbing work in both residential and commercial settings. This includes a range of activities, from fixture replacements to installing backflow preventers, as long as they meet the code's criteria for ordinary work.

  • Misconception 5: Submission of the OP128 Form Eliminates the Need for Further Compliance

A dangerous misconception is that once the OP128 form is filed, no further compliance with city regulations is required. In reality, submission of the form is just one step in adhering to NYC's plumbing regulations. Plumbers must ensure that their license and insurance are active at the time of filing and that all work complies with both the Department of Buildings and the Department of Environmental Protection regulations regarding asbestos abatement.

By dispelling these misconceptions, licensed master plumbers can navigate the reporting requirements more efficiently, ensuring that their work remains in full compliance with NYC's regulatory landscape.

Key takeaways

Filling out the NYC Buildings OP128 form, designed for reporting Ordinary Plumbing Work, is a crucial process for Licensed Master Plumbers to understand and follow correctly. Here are five key takeaways to guide you through the submission process:

  • Ensure all information is typewritten and the form is printed double-sided, adhering to the required format. This step is not just about clarity; it also aligns with submission standards, which can prevent unnecessary delays or rejections due to formatting errors.
  • The form requires comprehensive applicant information, including personal details and professional credentials, such as the business name, contact information, and the license number. Remember to accurately provide your full name, business address, telephone numbers, email, and the filing date, indicating month and year.
  • Location information is mandatory and must detail the work's specifics, including the borough number, block, lot, and BIN #, among other parameters. These details help categorize and track the status of plumbing work across the city accurately.
  • It's imperative to understand the scope of work permissible under the Ordinary Plumbing Work Report. This includes tasks such as minor repairs, replacements, or installation of plumbing fixtures within specified limits, removing or relocating parts of a domestic plumbing system, and more. Activities beyond this scope require different filing procedures.
  • Finally, the submission deadline and process are stringent. Reports must be submitted by the 15th of the month following the work's completion, with a $100 fee per report. Late submissions may result in violations or disciplinary actions. Ensure that the application includes a self-addressed, stamped envelope and is sent to the Department of Buildings ARA/Permit Renewals. Also, remember to attach a license number sticker, which can be obtained from the Plumbing Inspections office.

By keeping these key points in mind, Licensed Master Plumbers can navigate the OP128 form submission process more efficiently, ensuring compliance with NYC Building Code and avoiding potential penalties.
